Non-academic violations are behaviors that include, but are not limited to, alcohol, disorderly conduct, drugs, harassment, or other violations. Please use our online non-academic reporting form to file these reports.

Report an Academic Dishonesty Violation

Academic violations are behaviors that include, but are not limited to, cheating on tests, quizzes, or homework, plagiarism, collusion, or using unauthorized sources. Please use our online academic dishonesty violation form to file these reports.

General classroom behavior should be considered a non-academic violation. Please use the online non-academic reporting form to file these reports.

Report a Title IX Violation 

Title IX violations include dating violence, domestic violence, sexual harassment, sexual misconduct, or stalking behaviors. Please use our online Title IX reporting form to file these violations. 

Report a Title VI Violation

Title VI violations include discrimination against any individual on the basis of race, color, religion, national origin, sex, sexual orientation, age, disability, veteran status, or genetic information in matters of employment, admissions, housing, services, or its educational programs and activities. Please use our online Title VI reporting form to file these violations. 

Student Complaint Form

Students who have a complaint that they would like to be reviewed by Human Resources or the Dean of Students may submit that on the online student complaint form.

Report Concerning, Worrisome, or Threatening Behaviors

Faculty, staff, students, and guests are encouraged to use the online reporting form to report behavior that they perceive as concerning, worrisome, or threatening.
